Useful forms

We need lots of forms to do ANYTHING!! They always appear on the blog but if you don't get them straight away they can be difficult to find so you can find a collection here:-  Right click the image to see a high res image that you can print or download. Rifle Shooting

Here is our use of weapons form - the scout movement is a bit nervous about encouraging scouts to take up guns - we only do so in properly regulated ranges and for target shooting. This involves skill and concentration and is an Olympic sport at which Britain has had some success in the past. Here's a form for shooting, Boreatton have had scouts selected for the National Scout Squad.

One Day Activity Form

This form shouldn't be required unless you haven't completed the Survey Monkey Questionnaire here; http://boreattonscouts.blogspot.co.uk/2013/09/activity-permission.html or you have different contact details during the activity.

Shropshire Borders Scouts Mountaineering Group

This is the general form for hiking/climbing/caving organised or approved through SBSMG - I ask for a fresh copy each activity so I can give it to the leader of the day.

Scuba Diving

This BSAC form is needed each year before doing our regular scuba splash - Note if you have any of the notifiable medical history you must seek specialist advice from a BSAC doctor, A list can be found here; http://tiny.cc/zj34ax Unfortunately there is only one in Shropshire so unfortunately you may not be able to dive. (It's no problem to come with us and either watch or snorkel though).

Gift Aid Declaration

We can claim tax back on you subs if your parents may income tax. Please get them to complete this form.
